How to Enroll in Calibre Academy Online School in Arizona

Student’s Birth Certificate

A copy of the student’s birth certificate must be provided for every student to verify age eligibility. To avoid unnecessary delays, provide a copy of this document along with your enrollment documents.

Note: Birth certificates can be obtained from the Arizona Department of Health Services.

As a reminder, the age eligibility is as follows:

• Minimum Age: Students must be 5 years old by September 1.
• Maximum Age: Up to 8th grade (grade-age appropriate). 
  

Proof of Immunizations

Students must provide evidence of their immunizations on either a state of Arizona official immunization form, the Calibre Academy form, an official school record, a health care provider or health department printout or an immunization booklet with all doses stamped or signed.

Note: A health care provider must sign or stamp these forms.

Proof of Residency

You must reside in the state of Arizona and provide at least one form of acceptable proof of residency to be eligible for enrollment in Calibre Academy.

Note: United States citizenship is not a requirement.

Acceptable Forms of Documentation for Proof of Residency:

• State ID (must match home address)
• Utility bill (water, gas, electricity, sewer, internet/cable)
  • Note: Utility bill must be dated within 30 days of enrollment submission
• Rent receipt, residential lease agreement or mortgage statement
• Bank Statement
• Government correspondence

Prior Academic Records

Those who have previously attended public school, private school or homeschool can provide academic records for placement. Please provide a copy of an IEP along with the below, if it applies to the student.

K-8: Progress Report

Submit a progress report from the prior school year and any documentation issued for the current school year.
